of Present Illness : HISTORY: Patient was presented with complaint of off and on Breathlessness since last four and half month. She also had a history of back pain since long time. She also had a history of weight loss. She has taken treatment from Local Practioner but not relived. On examination her Pulse was 88 per Minute and BP was 120/80 mmhg. |
